* Please give us the name of your store or business, contact person, your telephone number, fax number, and e-mail address. Please give us your mailing address for U.S. Mail (we do not ship UPS). Please specify the quantity of each card you would like to order. Minimum order is 6 Altar Cards of any assortment.
* We will send your order to you immediately. We always send first orders on a “Net 10” basis. All subsequent orders will be sent “Net 30”. If you have a request other than “Net 10”, please let us know.
